For 7 years na kasal ako, hindi pa pala updated ung civil status ko sa PhilHealth! 😁  Same reason sa SSS kung bakit hindi ako nag-hulog agad-agad. ✌ Pero yun na nga, I updated my civil status and changed my employed status to voluntary. Went to PhilHealth SM North ( Annex Bldg., Lower Ground Floor, beside Bowling Center).  💡: Requirements needed if you're going to update your Civil Status and Name of Married Woman: ☑ Accomplished PMRF ☑ Photocopy of Marriage Certificate ☑ Present 1 Valid ID 💡 : You need to pay first your premium contribution before you change your employed status to voluntary. 💡 :  Payments can be made monthly, quarterly, semi-annually or annually. 💡: Requirements needed if you're going to change to Voluntary Member: ☑ Latest Payment Receipt ☑ Accomplished PMRF ☑ Present 1 Valid ID Nagbayad kami sa SM Business Center. Sa tabi lang din ng PhilHealth. Hingi kayo ng form kila ate cashier, sabihin nyo payment form/slip for PhilHealth. Image

How To Pay SSS Contribution If Your Are A Voluntary Member

I resigned from my job last April of 2017. So, after that, wala na kong hulog sa SSS. Sabi ko mag-huhulog kami ni Jules voluntarily. Pero months past, year past, wala pa din. Maybe my number 1 reason is nakaka-tamad mag-hintay. (We all know that!)  I don't know what urged me na mag-punta at mag-ayos sa SSS. We decided to go to SSS at SM North EDSA (Annex Bldg., Lower Ground Floor, beside Bowling Center). First, you need to write down your name at their Logbook .  FYI, they will not entertain you kahit may tanong ka lang, you still have to write your name sa logbook and remember your number, beside your name. And, our number called. We asked for an E4-Form (Self-Employed sana ia-apply namin). Sabi ni ate, kung gusto na lang daw ba namin mag-voluntary kasi same lang naman daw yun. We said yes. 😁  Ate asked for our SSS ID and how much yung contribution namin.  As per our last digit sa SSS number namin, pwede pa kami mag-bayad for the month of September. FYI again, h indi mo

Biliran-Leyte 2017 (November 28 - December 2)

Traveled  to Leyte with Hubby's Fam. Nanay's  hometown a nd for her birthday! 🎂 Day 1: Arrived in MNL Aiport at almost 9:30 am. Our flight was 11:00 am via Cebu Pacific . We boarded at exactly 11:00 am. Bus going to 🛧 ! Travel time is almost 2 hours from MNL Airport to Tacloban City Airport. Arrived at almost 1:00 pm We went straight to MacArthur Landing Memorial Park in Palo. The MacArthur Landing Memorial Park After picture taking 😁 we went to  Seafood & Ribs Warehouse Restaurant  to eat. Ang sarap ng baked scallops!! 😀😀😀 After mabusog we went to Carigara Port! Then we went to  Balai Constancia , where we stay throughout the trip.
