How To Pay PhilHealth Premium Contribution If Your Are A Voluntary Member | How To Apply For Senior Citizen

For 7 years na kasal ako, hindi pa pala updated ung civil status ko sa PhilHealth! 😁 Same reason sa SSS kung bakit hindi ako nag-hulog agad-agad. ✌ Pero yun na nga, I updated my civil status and changed my employed status to voluntary.

Went to PhilHealth SM North (Annex Bldg., Lower Ground Floor, beside Bowling Center). 

💡: Requirements needed if you're going to update your Civil Status and Name of Married Woman:
☑ Accomplished PMRF
☑ Photocopy of Marriage Certificate
☑ Present 1 Valid ID

💡: You need to pay first your premium contribution before you change your employed status to voluntary.

💡Payments can be made monthly, quarterly, semi-annually or annually.

💡: Requirements needed if you're going to change to Voluntary Member:
☑ Latest Payment Receipt

☑ Accomplished PMRF
☑ Present 1 Valid ID


Nagbayad kami sa SM Business Center. Sa tabi lang din ng PhilHealth. Hingi kayo ng form kila ate cashier, sabihin nyo payment form/slip for PhilHealth. Image below is my PhilHealth Premium Slip. 

I filled-out the PMRF, hiningi ko kay ate naka-assigned sa PhilHealth. I marked the box ✔ "For Updating" and so on. Image below is where I ✔ the Self-Earning Individual.

We also requested an ID. Pa-laminate na lang. Napag-iwanan na si PhilHealth. 😕 Binigyan din kami ng copy ng MDR namin.

I regret late na ko nag-hulog. Anyway, at least okay naman na. Ayos na. 😀

UPDATE:
I went again to PhilHealth SM North to accompany my mama.

It's so easy to apply for SC. First, photocopy SC ID. Second, ask for a form. Fill it out. No need to mark the Membership Category. Then give it to the PhilHealth Staff. After that, they will give you an MDR and an ID. They are all FREE.

SCs don't need to pay for their contributions because their source of contributions are from the Sin Tax Law.
